Off Sick for 7 Days or Less
You do not need a sick note from your GP. You employer should ask you to complete a self-certification form when you return to work.

First Sick Note for a New Illness
If this is your first sick note for a new illness or condition, you’ll need to book an appointment with a clinician to discuss your symptoms.
You can book an appointment by:
- Login into the NHS app
- Using SystmOnline
- Calling us on 01332 332 628
Important: Please Note
You can self-certify for the first 7 days of your illness. A sick note is only required after day 7. If you are unable to book an appointment straight away, please keep trying. We will do our best to see you as soon as possible.
Requesting an Extension to Your Sick Note
If you feel you need more time off due to illness, you can request an extension to your current sick (fit) note using the form below.
Once we receive your request, the clinician who issued your last note will review your medical records. Based on that review, they will either:
- Issue an extension to your current sick note.
- Ask you to book an appointment for a review.
- Advise that an extension is not needed.

Sick notes can be backdated if necessary and appropriate. If you’re unable to book an appointment straight away, please keep trying and we’ll do our best to see you as soon as possible.
