Travel risk assessment

Use this service to request a travel risk assessment with a practice nurse.

You can use this service if you:

  • are registered at the surgery
  • are traveling abroad in at least 8 weeks
  • are traveling to a maximum of 3 destinations

If you are traveling abroad in less than 8 weeks, you will need to contact a local pharmacy to see if they can vaccinate you sooner. The pharmacy may request a copy of your vaccination records. You can request these using the medical report request form.

If you are traveling to more than 3 destinations, you will need to contact a community pharmacy or travel clinic to arrange your travel vaccinations.

Before you start

We’ll ask you for:

  • your first and last name, date of birth, sex, postcode, email and phone number
  • if applicable, the details of the person you are completing the form on behalf of

If multiple people are travelling with you, you will need to complete this form separately for each person who is registered with us.
