Self-refer to midwives

Use this service to self-refer to the midwife service for care during your pregnancy.

Please note that this service is not for urgent medical enquires. If your symptoms worsen or your need medical help more urgently, contact NHS 111 or 999.

You can use this service if you:

  • are registered at the surgery
  • are currently pregnant, or have reason to believe that you are currently pregnant

Before you start

We’ll ask you for:

  • your first and last name, date of birth, sex, postcode, email and phone number
  • if applicable, the details of the person you are completing the form on behalf of
  • details of your last menstrual period
  • details of any pregnancy tests you have done
