Requesting Your Medical Records

If you need information from your medical records, this guide will help you get what you need quickly and at the lowest cost.

Important Notes:

  • Some requests are considered private work and may include a fee. We’ll let you know if this applies.
  • Keep copies of any documents we give you. If you ask for the same information again, there’s a £70 duplicate request fee.

Choose the Reason for Your Request:

If the reason for your request is not listed below, please complete the medical report request form and we will try our best to provide you with the required information.

1. PIP / Universal Credit

You don’t need to request anything from us. Your application form will say not to contact your GP.

Use the NHS App or online services to check your summary care record if you need help remembering dates or diagnoses.

The Department for Work and Pensions will contact us directly if needed.

2. Taking Medication on a Plane

Most medications only need a copy of your prescription.

Check the government website for travel rules.

Some items (like insulin needles) need a doctor’s letter. This is private work and will be charged based on the time it takes.

If you need a letter, fill out the medical report request form.

3. Proof of Diagnosis

You can request a patient summary, which includes:

  • Current medications
  • Known allergies
  • Active problems
  • Recent consultation notes

It takes up to 4 weeks to prepare. Request it using the medical report request form.

4. Taxi Medicals

This is private work and costs £200.

You’ll have an appointment with a doctor who will complete the evaluation and forms.

If a private provider is doing your medical, we can give you a patient summary (allow 4 weeks).

Use the medical report request form to request either.

5. Fitness to Work / Employer Medical Report

Our doctors are not trained in occupational health, so we can’t assess your fitness to work.

Your employer must use their own occupational health provider.

If they need medical info, they must request it from us with your written consent.

They can only ask for specific details – not your full record.

6. Proof of Address

We don’t provide proof of address letters.

We can give you a letter of registration (cost: £20 per patient).

This includes:

  • Your name and address
  • Date of registration
  • Signed by admin staff (not a doctor)

Check if the organisation will accept this before requesting.

7. Letter of Registration

Same as above. Cost is £20 per patient.

Request it using the medical report request form.

8. Insurance Report

We charge £104 for this.

If your insurer requests it, they pay. If you request it, you pay.

You have the right to view the report before it’s sent.

Return it quickly—delays may cause your insurer to cancel the request.

9. Moving to a New Doctor
  • Within the UK: No need to request records—they transfer automatically.
  • Outside the UK: You can request a copy of your medical record via the medical report request form.
10. Court or Solicitor Requests

These take about 4 weeks to prepare.

Request early to ensure you have everything in time.

11. Full Medical Records

If you need a full copy of your medical records, you can request this through our website. Please note:

  • It can take up to 4 weeks to prepare.
  • You may be asked to verify your identity before we release the records.
  • If you’ve already received your records and request them again, a £70 duplicate fee may apply.

Make your request using the medical report request form and clearly state that you are asking for your full medical records.
