New Patient Registration

The doctors welcome new patients who live within our practice area (please click on the Practice Area tab to see if you live in our practice area). 

As it often takes sometime for records to be forwarded from your former practice, all newly registered patients will be asked to complete a health questionnaire and book to see one of the Practice  Nurses for a New Patient Medical.

Medical treatment is available from the date of registration. 

If you would like to register with our Surgery and live in our practice area, please call in at Reception to complete a Patient Registration Form. You will need to provide Identification (namely a Birth certificate/passport and a Utilities Bill showing your name and your current address) to register with us.


Named GP for all patients

As part of the new 2015/16 General Medical Services contract, all our patients  have now been allocated a named GP and this GP will be responsible for your overall care at the practice.

So what does this mean for you?

All our patients are allocated a named doctor. This does not mean you can only see that GP. You are able to continue to choose the doctor you wish to see or the GP who is available on the day. Therefore you should see no change in the day to day running of the practice or your care.

However, if you do not know who your allocated named GP is, please feel free to ask the Receptionist when you next visit the surgery.

 If you would like your allocated named GP changing we will make reasonable efforts to accommodate your request.

Please remember this DOES NOT affect which doctor you usually see at the practice.
